Services

Baxter Palliative Consulting is an independently owned practice providing specialty palliative services to patients with serious illness. Comprehensive palliative care includes discussion/education on chronic and serious illness as it relates to an individual’s function and quality of life.  The service offers chronic illness education, symptom management, advance care planning and completion of healthcare directives. This specialty service does not take the place of a primary care provider. Patients will continue to be managed by their primary care provider. It is a service that will support primary care providers in caring for patients with chronic or serious illness.

Who Can Benefit?

  • Anyone who has the presence of chronic or serious illness

  • Aging adult with healthcare declines and distressing symptoms

Examples may include:

  • Individuals receiving cancer treatment who have difficult symptoms such as pain, nausea, or anxiety

  • Individuals with advanced COPD who are experiencing fatigue, shortness of breath, or insomnia

  • Individuals with dementia who are experiencing declines in function, cognition and appetite

Where do we meet?

Meetings can be held in our office or in the patient’s home if preferred. Additionally, we offer palliative care services by telehealth via a secure Zoom platform which is HIPPA compliant. This option affords the ability to connect patient, caregiver, and care providers for our conversations.
